Home Care Programs
Minnesota's Alternative Care Waiver and ICLS programs fund home care for eligible seniors and adults with disabilities. Triple N is an enrolled provider in both programs serving the Twin Cities metro.

Minnesota’s Alternative Care (AC) Waiver provides home care for seniors 65+ who need nursing-level support but don’t yet qualify for the Elderly Waiver. ICLS supports adults with physical disabilities to remain living in the community. Triple N Home Health Care is enrolled in both programs and serves eligible individuals throughout the Twin Cities metro. Call (952) 898-4237 to check eligibility.

Alternative Care
AC Waiver
Who: Adults 65+ not yet eligible for Elderly Waiver
Funded by: Minnesota state funds (not Medicaid)
Purpose: Delay or prevent nursing home placement
Administered by: County Social Services
Intensive Community Supports
ICLS
Who: Adults 18+ with physical or developmental disabilities
Funded by: Medical Assistance and state funds
Purpose: Support independence in the community
Administered by: County Social Services

- Companion care and social support
- Homemaker services — meal prep, housekeeping, laundry
- Respite care for family caregivers
- Transportation and errand support
- Personal care assistance and ADL support
- Night supervision and safety monitoring
