Minnesota's Alternative Care Waiver and ICLS programs fund home care for eligible seniors and adults with disabilities. Triple N is an enrolled provider in both programs serving the Twin Cities metro.

What are Alternative Care and ICLS and does Triple N provide them?

Minnesota’s Alternative Care (AC) Waiver provides home care for seniors 65+ who need nursing-level support but don’t yet qualify for the Elderly Waiver. ICLS supports adults with physical disabilities to remain living in the community. Triple N Home Health Care is enrolled in both programs and serves eligible individuals throughout the Twin Cities metro. Call (952) 898-4237 to check eligibility.

ICLS
Who: Adults 18+ with physical or developmental disabilities
Funded by: Medical Assistance and state funds
Purpose: Support independence in the community

How to Enroll

Contact Your County
Reach out to your county social services office to request an assessment for AC or ICLS eligibility.

Complete an Assessment
A county social worker conducts a needs assessment to determine eligibility and the level of support authorized.

Choose Triple N
Select Triple N as your enrolled provider. We complete intake, build your care plan, and assign your caregiver.
