What Is the CFSS Program?
CFSS gives eligible Minnesotans the funding and flexibility to direct their own home care.
Community First Services and Supports (CFSS) is a Minnesota Medicaid waiver program administered through Medical Assistance. It replaced the Consumer Support Grant and Personal Care Assistance programs, giving individuals with disabilities more control over how their home care is delivered.
Under CFSS, eligible individuals receive a budget based on their assessed support needs. That budget pays for an approved provider — like Triple N — to deliver care. You decide what support you need, when it happens, and what your goals are. Triple N handles the employment side.
Who Is Eligible?
- Individuals of any age — children, teens, adults, and seniors
- Enrolled in Minnesota Medical Assistance (Medicaid)
- Have a disability, chronic condition, or functional limitation
- Require assistance with activities of daily living
- Must complete a county or state CFSS assessment
What Does CFSS Cover?
- Bathing, dressing, grooming, and personal hygiene
- Mobility assistance and transfers
- Meal preparation and feeding assistance
- Light housekeeping, laundry, and errands
- Community access and participation support
- Skill-building and individualized supports (IHS-like activities)
- Supervision and safety monitoring
Agency Model vs. Budget Model
CFSS offers two delivery options. Triple N operates as an Agency Model provider. This means Triple N employs, trains, schedules, and supervises all caregivers on your behalf. You direct what care happens — Triple N handles all employment responsibilities. This is ideal for families who want professional infrastructure without managing employment themselves.
